| Bill Payment Options |

| At Mid-Hudson Co-Operative, we have many different payment
options available to you. We work just as hard to make paying your premiums
easy. Choose from variety of payment options below make a one-time payment
or pay in installments. Or you can choose your method of payment by Credit
Card (Visa, MasterCard or Discover Only), our direct draft service, or personal check
or money order via the mail. |

| Payment Plans-Pay When You Like |

| You can Choose - One Payment For The Full Premium |
| |
| Save money - No service charge will be applied by making
just one payment for the full premium instead of installments. We'll bill
you for the full amount of your premium a few weeks before your renewal
date. You don't pay any installment service fees. |

| Choose An Installment Plan if your premium is over $300
to Suit Your Budget |

| There is a $5.00 per installment Fee. |
| |
| 4-Pay-Pay Your Premium in Four Installments |
| |
- Pay 25 percent by the effective date of your policy.
- Pay 25 percent + installment fee will be due 90 days after the policy
effective date.
- Pay 25 percent + installment fee will be due 180 days after the policy
effective date.
- Pay 25 percent + installment fee will be due 270 days after the policy
effective date.

| 5-Pay-Pay Your Premium in Five Installments |

- Pay 20 percent by the effective date of your policy.
- Pay 20 percent + installment fee will be due 60 days after the policy
effective date.
- Pay 20 percent + installment fee will be due 120 days after the policy
effective date.
- Pay 20 percent + installment fee will be due 180 days after the policy
effective date.
- Pay 20 percent + installment fee will be due 240 days after the policy
effective date.
*Note: There will be a service charge fee for any dishonored checks
returned. The fee imposed depends upon the schedule of rates, fees and charges
utilized by our current depository bank. This fee shall not exceed $30.00 |

| Cancellations for non-payment of premium will be issued when
a check has been dishonored by the bank or no payment has been received
on its due date. The amount due will be the original premium, plus any late
fees. The amount payable will be payable in cash, check, money order, certified
bank check, Credit Card or Agency Check. |

- Simply Download the Mid-Hudson
Direct Draft Form and follow the Instructions on the form.
- Once we have set up an account, your payments will be deducted automatically
from your checking account.

| Here's How it Works |
| |
- We'll send you a bill approximately two weeks before each payment
is due, along with an pre-addressed envelope for sending your check
or money order.
- Mail us your payment, enclosing your paper bill stub. Please remember
to write your policy number on the check or money order.
- Make sure that you mail your payment in time for it to reach us by
the due date.

Pay your Bill Online or By Phone with a Credit Card  |

You can now pay your Mid-Hudson bill online with either a Visa, MasterCard
or Discover (Accepted
Only), by simply clicking on the "Online Bill Payment Button"
to your left. You
will be taken to our secure website and you can be assured that no information
about you or any credit card information will be shared with
anyone. You may also choose to Pay with a Credit card by Phone as well,
and you may dial 1-800-721-5001 (Toll free for New York State
Residents only) or 845-457-5001. Simply ask for the accounting
department and they will be happy to assist you. |
